CASE PREPARATION / YOURABLECASE
Your medical story needs more than a diagnosis.
SSA needs evidence that explains the nature, severity, and functional impact of a condition. Here is how to get organized before a detailed case review.
THE GUIDE / 01
What medical evidence can show
A diagnosis tells SSA what condition you have. Medical evidence can also show the symptoms, objective findings, treatment history, limitations, and likely duration. An applicant’s ability to function and work is important.
Treatment history
Provider visits, specialist care, procedures, therapy and medication changes.
Clinical findings
Exams, labs, imaging and other testing appropriate to the condition.
Function over time
How the condition affects standing, lifting, concentration, attendance, persistence or other work activities.
THE GUIDE / 02
Make an evidence timeline
A simple written timeline may help you recall your symptoms and the care you received. Aim for accuracy rather than trying to make a diagnosis sound more serious.
- Approximate month and year symptoms began interfering with work
- Provider names, clinic locations and appointment dates
- Hospitalizations, surgeries, therapy and medication changes
- Days missed from work, job adjustments and reasons for leaving work
- Important changes since a prior SSA decision
THE GUIDE / 03
What to avoid
Do not send Social Security numbers, full medical records, diagnostic images or payment account data through the website contact channels. If the firm accepts your matter, staff can explain secure document collection options.
Do not exaggerate, alter, or withhold relevant facts. Accuracy and consistency matter at every stage of the process.
THE GUIDE / 04
When a case reaches a hearing
Medical evidence may need to be submitted under hearing-level rules and deadlines. A representative can explain what additional documentation is relevant and how it should be submitted to SSA when a formal appointment exists.
QUICK ANSWERS
Questions worth asking.
Do I need every record before requesting a review?
No. Start with your stage and basic details. If the firm can assist, it can explain what information is needed next.
Can I upload records to this public website?
The public intake deliberately does not accept medical uploads. Only use a verified secure method arranged with the firm.
YOUR NEXT MOVE
